Transcutaneous Bilirubin Measurements in Neonatal Jaundice

Abstract
Neonatal hyperbilirubinemia is probably the most frequently encountered diseases in neonates and may induce kernicterus. So it is important to screen and treat the pa ient early.
To date, repetitive heel pricks and continuing blood sampling has been used to evaluate the serum bilurubin, but it may serve as a source of discomfort and of serious infection Recently transcutaneous bilirubinometry was introduced and has been used.
The present study was performed in 263 neonates from June 1984 to Feb. 1985 at Kangnam St. Mary¢¥s Hospital to evaluate the accuracy and precision of the transcutantoun bilirubinometry and several factors that were assumed to influence the numerical index. The results were obtained as follows.
I) The precision and technique dependence in transcutaneous bilirubin measurements as relatively accurate and easy.
2) Coefficients of correlation between total serum bilirubin concentration levels and the transcutaneous bilirubin measurements of different body sites were notably high in abdomen, which can be expressed as: r=0.800, Y=0.66X+8. 16, p<0.001.
3) Coefficients of correlation between total serum bilirubin concentration levels and the transcutaneous bilirubin measurements of three different Ponderous index groups showed most reliability in Ponderous index 2.3-2.55 group, which can be expressed as: r=0.848 others r=0.807, r=0.805.
4) Coefficients of correlation were 0.690 at phototherapy group and it showed poor reliable relation rather than control group.
5) Coefficients of correlation were 0.710 at meconium-stained group and it showed to lower relation- than control group.
6) Coefficients of correlation lower serum bilirubin concentration group(below lOmg/dl) ti -re higher than that of high serum bilirubin concentration group (above 10mg/dl), respectively 0.849, 0.577.
